Case management model analysis of the health system context

Abstract
The aim of the article is to highlight and assess the case manager‘s role in improving performance of health system, the benefits of patients, doctors and nurse, and the influence on quality of services.
Analysiss existing organization of primary health care problems and an overview of the Lithuanian health system development framework for 2011-2020, approved by Lithuanian Parliament Seimas, in which problems in the PSP is proposed for case management. In practice, this is still not widespread, and the health institutions care is a novelty. The aim was to try and check usefulnessthe, so case manager project was launched in the Centro Out-patient clinic Performed analysis of the role of case manager will disclose whether it is good, useful and effective.
